Your post addressed what I believe to be a key intervention in planning (keeping patients from “falling through the cracks” in the healthcare system) for Mr. Monsoor’s care—providing care and support through in my words would be through communication and collaboration with multidisciplinary team members in various supportive roles within the community! To tie this into this week’s lesson, nurses working in Community Nursing Practice roles also serve an important function in ensuring continuity of care once a patient is transitioned from an acute healthcare facility to out into the community setting. FYI, the need for a focus on interdisciplinary collaboration and communication began in the early 2000s, with the 100,000 Lives Campaign initiative, launched by Dr. Donald Berwick in 2004 through the National Institute of Health. When it came to the forefront that the various of healthcare providers were not communicating across disciplines with each other, resulting in a number of errors being made that contributed to decreased patient health outcomes and death. Soon afterwards came the 5,000,000 lives campaign, with a focus on reducing harm caused by healthcare workers. Just think of how many patients’ lives have been saved, and/or harm caused by, for example, wrong site surgeries, has been reduced from this one initiative to improve interdisciplinary communication among healthcare workers!
