After achieving adequate sedation, the oral cavity was anesthetized with lidocaine. The endoscope was introduced through the mouth and advanced under direct visualization into the esophagus, stomach, and duodenum. The esophagus was inspected from the gastroesophageal junction to the upper esophageal sphincter. Notable findings included large, tortuous varices present in the distal esophagus. Endoscopic variceal ligation (EVL) was performed using rubber band technique for the varices in the distal esophagus. Three varices in the distal esophagus were successfully banded without complication. The stomach and duodenum were also examined. No significant pathology was noted in the stomach or duodenum, except for mild gastritis, which was biopsied. The procedure was well tolerated, and hemostasis was achieved. The endoscope was then carefully withdrawn. Findings: Esophagus: Large esophageal varices in the distal esophagus. Stomach: Mild gastritis, portal hypertensive gastropathy Duodenum: Normal. Procedure: Successful placement bands in the esophagus. No active bleeding noted. What are the correct diagnosis and CPT procedure codes for this case?
