SBAR ANNA SECORD Situation: Anna Secord, a 77-year-old female, is presenting with [briefly describe the current issue, e.g., “shortness of breath and chest pain”]. Background: Anna has a history of hypertension, chronic obstructive pulmonary disease (COPD), delirium, dementia, urinary incontinence, and an indwelling catheter. She also has behavioral symptoms, a history of falls, pressure ulcer injury, and is on psychotropic drugs. She was admitted to the hospital two days ago for exacerbation of COPD. Her current medications include atorvastatin calcium 20 mg, polyethylene glycol 3350 powder, and quetiapine fumarate ER 200 mg and 150 mg. She has a known allergy to penicillin. Her diet is regular. Assessment: Her recent vital signs are BP: 117/61, RR: 18, pulse: 60, blood sugar: 121, O2: 95%, and pain level: 0 on a scale of 1-10. Recommendation: I suggest starting Anna on supplemental oxygen therapy to keep her oxygen levels stable. It would also be a good idea to get a chest X-ray to check for pneumonia or other lung issues. Given her complex medical history, we should review her psychotropic medications to make sure they’re not causing any problems. We need to manage her indwelling catheter carefully to prevent infections and put fall prevention strategies in place to keep her safe. Additionally, we should monitor her pressure ulcers and provide the necessary wound care to help them heal. Medications: Atorvastatin is medication used to lower cholesterol and triglycerides (fats) levels to help prevent heart disease, angina (chest pain), strokes, and heart attacks Polyethylene glycol 3350 is used to relieve constipation. This medicine is a laxative. Quetiapine is used to treat schizophrenia in adults and children who are at least 13 years old. It is also used alone or with with divalproex or lithium to treat episodes of mania (frenzied, abnormally excited or irritated mood) or depression in patients with bipolar disorder (manic depressive disorder; a disease that causes episodes of depression, episodes of mania, and other abnormal moods).
