could you help me answer these questions regarding a problem I encountered as a student nurse at clinicals? My problem was a physiological issue with a patient. That patient had AIDS, sepsis and had altered mental status. My nurse did not update me on any of those issues prior to the start of my rounds and I did not have time to read the notes in the EHR.
