DESCRIPTION OF PROCEDURE: The patient was taken to the OR and general anesthesia was administered. He was intubated with a double lumen endotracheal tube and placed in a right side up lateral decubitus position. T he chest was prepped and draped in a routine fashion. Posterolateral thoracotomy through the fifth intercostal space was carried out. We could easily feel the mass in the upper lobe, in the mid portion of the upper lobe. We dissected out the paratracheal nodes and sent those for frozen. Those came back with no evidence of any malignant change. We then proceeded to dissect out the pulmonary artery branches to the upper lobe and these were doubly ligated with silk sutures and clips divided. We divided the vein using the vascular stapler. We then divided the incomplete fissure between the upper lobe and middle lobe, using the ILA stapler. Before firing the staples, we checked to make sure we had good aeration in the middle and lower lobes. Once we fired the staples, we removed the upper lobe to Pathology. Our margins were free of tumor. We checked for an air leak up to 40 cm of pressure. None was seen. We checked for hemostasis. We placed two 36 French tubes through separate stab wounds and sutured to the skin with silk suture. We approximated the ribs with heavy intercostal Vicryl sutures that were tied and cut. We used a running, heavy Vicryl for all the muscle layers, some running Vicryl for the subcutaneous tissue and running subcuticular Vicryl for the skin. All instruments, sponge and needle counts were correct. The patient tolerated this procedure well. Blood loss was about 500 cc. What is the ICD-10-PCS code(s)?
