Patient Case: Jerry Jerry is a 2-month-old white male infant of Northern European descent who lives with his mother, father, and 8 -year-old half-sister. Both parents smoke in the home. English is the primary language spoken by his family. Further analysis reveals no family history of CF. Jerry was born at full term, with a normal spontaneous vaginal delivery, weighing 8 pounds Nounce at birth (50th percentile). Maternal iron deficiency anemia complicated the pregnancy. He went home on day 2 of ble, at which time he experienced non-projectile emesis after every feed. No weight gain was noted despite trials on various infant formulas including Enfamil (cow’s milk based), isomil (soy based), and Nutramigen (elemental formula). Despite a voracious appetite, at 3 weeks of age, Jerry weighed 7 pounds 14 ounces, and at 8 weeks of age, he weighed 9 pounds, below the 5 th percentile for weight. The parents reported he had frequent yellow, greasy stools. Jerry was referred to the hospital at 2 months of age for work-up related to poor weight gain and severe gastroesophageal reflux (GER). Initially diagnosed with failure to thrive, a sweat test revealed a sodium level of-150 mg, thus confirming his diagnosis of CF. Jerry and his family were referred to physical therapy for family instruction in airway clearance techniques. He has had no haspitalizations from the time of his initial diagnosis at 2 months of age until 8 years of age.
